We are currently updating forms on the website, please contact us before downloading any forms to ensure you have the most current info. If you submit a form that is not the current document you will be asked to resubmit the correct form.
If you are a returning family, please complete the commitment form.
New families need to complete the application.
We are a parent volunteer homeschool co-op. Parents are required to attend.

Now the piece everyone is waiting for.... Registration! Be on the lookout for the email entitled: Registration Instructions for the link to submit your family and student registration surveys during registration week.
Here are the forms you will need for Mandatory Meeting:
FAMILY CONSENT AND
RELEASE FORM
(turned in at Mandatory Meeting once per school year)
BACKGROUND CHECK FORM
(turned in at Mandatory Meeting your first semester, then every 3 years after that)
FAMILY EMERGENCY MEDICAL FORM
(turned in at Mandatory Meeting once per school year. If attending both Fall and Winter/Spring, you'll simply sign off on the one from the Fall)
Statement of Faith
(turned in at Mandatory Meeting just once; we require an original, signed statement of faith on file)
