We are currently updating forms on the website, please contact us before downloading any forms to ensure you have the most current info. If you submit a form that is not the current document you will be asked to resubmit the correct form.

Please remember to save your completed forms as a PDF prior to submission. 
Every computer is different.  Try clicking the print command and then click on the "change destination." There should be an option to "save as a pdf."

CLASS REGISTRATION FORM (WORD)
(turned in ON your registration day - never before- every semester)

CLASS REGISTRATION FORM (PDF) 
(turned in on your registration day every semester. Use this if you aren't able to use the Word file)
